Utah Crash Reports

We'll Find Your Crash Report

Tell us what you know about the crash. We'll track down the report and send it to you.

Request your crash report

A report number helps, but you don't need one. The date, the place and a name are usually enough.

If you give us an email address, we can send the report there.
MM slash DD slash YYYY
Your best guess is fine.
It is usually on the card or slip the officer gave you. Leave it blank if you don't have it.
Your part in the crash(Required)
Was anyone hurt?

What we need

The date of the crash and where it happened. The report or case number if you have it, which is usually on the card or slip the officer gave you.

What happens next

We find the agency that took the report, request it with your permission, and contact you when we have it.

No commitment

Asking for your report doesn't commit you to hiring us.

Why the form asks for your permission

Crash reports in Utah are protected records. The agency releases the full report to the people involved in the crash, the people hurt in it, and their agent, parent, or legal guardian, and the law counts anyone with your signed permission as your agent (Utah Code § 41-6a-404). The box at the end of the form is that permission, and it covers only the report for your crash.

If you are asking about a crash a family member was in, they need to give their own permission unless you are their parent or legal guardian. Tell us in the form and we will sort it out with you.

Asking does not make you our client

Sending this form does not hire us and does not commit you to anything. If you were hurt and want to talk about a claim, we are glad to. Free consultation. No attorney fees unless we win. Hablamos Español.

Prefer to request it yourself?

You have the right to request the report directly from the agency. Our guide on how to get your Utah crash report walks through it, and we have agency guides for the Utah Highway Patrol, Salt Lake City Police, Unified Police, West Valley City Police, Sandy Police, Provo Police, Orem Police and the Utah County Sheriff.
